I’m not an expert by any stretch, but it looks fairly legit.
You can see the lighting of the picture is similar to the lighting of the scene/world, including some mild reflection of the ground and cars in front of the dark parts of the picture (his black jacket).
These are very subtle details that, while always possible to fake, would have been a pretty serious investment in time and energy to have done.
Is this an actual ad or Photoshop
The fact that there is a shadow from the frame on all sides makes me think its edited.

And put it up in conservative areas (e.g. suburban freeway billboards), not the middle of city.
That may not have the effect you seek
As far as I can find online, it’s real. Would have to have a New Yorker go to 5th Ave. and 49th St. to verify, assuming it’s still up.

So I’m inclined to think it’s legit.
